STUDENT INSURANCE COVERAGE FOR ACADEMIC YEAR 2011/2012

Freshmen 2011– 1 July 2011 to 30 June 2012

Eligibility

  • Full-time local and international undergraduate students
  • International exchange students (excluding local exchange students)
  • Other students who have paid the insurance premium (Incorporated in the fees)

PERSONAL ACCIDENT COVERAGE

  • Accidental Death — S$30,000 per student
  • Accidental Permanent Disablement — up to S$45,000 per student
  • Accident Medical Reimbursement — S$5,000 per student
  • Physiotherapy Reimbursement — $750 per student within Personal Accident of $5,000 limit
  • 24-hour worldwide coverage

Claim Procedures

  • Claim form must be submitted within 30 days after each treatment
  • If the above is not possible, please give a notice of claim via email within 30 days after each treatment and follow-up with your supporting documents for claims when available
  • Expenses shall be reimbursed by AXA Insurance Singapore Pte Ltd.
  • A referral letter from your GP (for Specialist Treatment) or your Specialist (for X-ray & Diagnostic Services) must be obtained
  • Submit your duly completed claim form, all original bills/receipts, hospital discharge summary (for hospitalisation, surgery or day surgery done at government/restructured hospitals) and the aforesaid referral letter to the Office of Student Life
  • Upon receipt of all relevant documents by the insurer, reimbursement for approved claims will be credited into the student's bank account within 2–3 months

OUTPATIENT GP CONSULTATION COVERAGE

  • Your visits to the SMU City Campus clinic, approximately 200 other clinics (click here for panel listing) and the Government polyclinics are reimbursable
  • You can claim for consultation, standard medication and treatment

Claim Procedures

For Referrals to a Government/Restructured Hospital for Specialist Treatment and/or Hospitalisation/Surgery

  • If you are referred by the SMU City Campus clinic or a Panel Clinic, you are considered a Private patient.
  • To qualify as a Subsidised patient, you must be a Singaporean or Singapore PR,  referred by a polyclinic and without reference to a Specialist by name.

SMU City Campus Clinic and Panel Clinics

  • Produce your SMU student card upon registration
  • No payment is required at the clinic for consultation and standard medication for a insurance covered medical condition
  • You may have to top-up for non-standard medicine

Polyclinics and Hospital A&E Department

  • You are required to submitted your claim within 30 days after each treatment
  • If the above is not possible, please give your notice of claim via email within 30 days after each treatment and follow-up with your supporting documents for claims when available
  • Expenses shall be reimbursed by AXA Insurance Singapore Pte Ltd
  • Submit the duly completed claim form and all original bills/receipts to the Office of Student Life
  • Upon receipt of all relevant documents by the insurer, reimbursement for approved claims will be credited into the student's bank account within 2–3 months

OUTPATIENT SPECIALIST CONSULTATION COVERAGE

  • Outpatient specialist, X-ray and diagnostic services are reimbursable, on the condition that:
    • the treatment is recommended by our listed panel of general practitioners or
    • it is a follow-up treatment as referred by SMU's previous appointed medical provider

  • If the above is not possible, please give notice of claim via email within 30 days after each treatment and follow-up with your supporting documents for claims when available
  • Expenses shall be reimbursed by AXA Insurance Singapore Pte Ltd.

HOSPITALISATION & SURGICAL COVERAGE

Hospitalisation expenses incurred in government/restructured hospitals.

Claim Procedures

  • Your claim form must be submitted within 30 days after each treatment
  • If the above is not possible, please give notice of claim via email within 30 days after each treatment and follow-up with your supporting documents for claims when available
  • Expenses shall be reimbursed by AXA Insurance Singapore Pte Ltd
  • Submit your duly completed claim form, all original bills / receipts / admission form / hospital discharge summary and referral letter to the Office of Student Life
  • Upon receipt of all relevant documents by the insurer, reimbursement for approved claims will be credited into the student's bank account within 2–3 months

Hospital Admission (Letter of Guarantee)

  • A Letter of Guarantee (LOG) can be issued for hospitalisation/surgery that is likely to be covered under the insurance scheme
  • With the LOG, the cash deposit will be waived at admission and the insurer will settle the hospital bill directly with the hospital, up to the limits of the insurance coverage
  • Please forward the Letter of Admission/Day Surgery Authorisation Form issued by the hospital during financial counselling to MYCG at least 2 days in advance to request for the LOG
  • For emergency situations, please contact MYCG as soon as possible to request for the LOG
  • If a LOG is issued, please submit the Letter of Indemnity with the claim documents. Please refer to the FAQ for more details
  • Please note that the insurer reserves the right to review the claim documents submitted to determine if the medical expenses are covered
  • LOG will not be issued for medical treatment that is likely not covered
  • Please be highlighted that for local students, as their coverage is $50/day for room & board and sub-limits for surgery and other hospital expenses, they are likely to have to pay the cash deposit/settle part of the bill if they stay in a higher ward or their medical costs exceed the policy limits. This will be determined by the hospital.
