Part 2: Checklist for Verification & Card Collection Day

Kindly come down to SMU in person during the IDs Verification & Card Collection period with the following documents:

Note: Only students who have matriculated and submitted their photograph online are eligible to attend the IDs Verification & Card Collection Day!
If you have matriculated in window 1, then you should attend the 1st IDs Verification window.

*2.	Pre-Enrolment Medical Report (RMG)	Compulsory for Verification
3.	Indemnity Form (only applicable to student below 18 years old, please refer to the instructions under Applicant's Self Service > Online Matriculation > Matriculate Online)	Compulsory Submission if below 18

* Items 1 & 2 are not applicable for previously matriculated NSMen who had collected their SMU student card from Office of Undergraduate Admissions (SMU Admin Building, Level 3 / Tel: 6828 0980).
